Fees & Services

The Ontario Ministry of Health and Long Term Care (OHIP) no longer covers medical services and procedures that they consider medically unnecessary. Patients are therefore required to pay for any such services and procedures that are performed by a physician.

The following list represents some of the services that can be offered to you at this clinic, as well as their current fee (based on the Ontario Medical Association of Fees). Payment for service is due on the day of treatment unless prior financial arrangements have been made with our office.

Off work note – $20.00

Forms (extra charges may apply for examination):

Trip cancellation insurance forms – $35.00
E.I.C. disability/maternity certificate – $20.00
Federal Disability Tax Credit form – $45.00
Disability insurance forms (per page) – $28.00

Third party medical examinations:

Includes completion of forms – $30.00
Driver’s license physical – $125.00
Insurance physical – $150.00
Work / school medical – $60.00
Fitness club physical – $60.00

Travel medical advice/consult – $35.00

NSF cheque – $20.00

Mantoux TB skin test (except when medically necessary)

1 step – $24.00
2 step – $35.00

Missed appointment (without prior notice)

15 minute minor appointment – $25.00
30 minute major appointment – $50.00

Physician authorization for insurance coverage

Massage, chiropractor, orthotics, etc – $20.00
Medical devices not covered by OHIP, but physician prescribed – $20.00

Telephone medication

Repeats when not seen by a physician – $20.00

Faxing prescriptions

Per patient request – $5.00

Liquid nitrogen treatment (up to 5 lesions/treatment)

Warts (except plantar/genital – OHIP covered – $20.00
Seborrhelic keratoses – $20.00
Other benign lesions – $20.00

Removal of benign lesions (oles, skin tags, etc. by excision and suture)

Single lesion – $50.00
Two lesions – $60.00
Three lesions – $70.00

Medical records (per patient)
